NCSTA Awards Nomination Form

Nominations must be submitted by July 31!

In addition to the form below, the following items are also required for each nomination and may be submitted online as well:

  • A narrative from the nominator describing the nominee's experience and/or contributions. Limit to 500 words.
  • A minimum of two (2) and a maximum of five (5) letters of support. One must be from the educator's principal or supervisor. One must be from the student teacher’s mentor teacher.
