NCSTA Awards Support Letter Form

Nominations must be submitted by July 31!

The following form may be used to provide these components of the nomination packet:

  • A narrative from the nominator describing the nominee's experience and/or contributions. Limit to 500 words.
  • A minimum of two (2) and a maximum of five (5) letters of support. One must be from the educator's principal or supervisor. One must be from the student teacher’s mentor teacher.
